WOODMERE, NY — The Five Towns Premier Rehabilitation & Nursing Center has completed what facility leadership describes as an outstanding state survey, a development management believes could help position the Woodmere center for a higher federal star rating.
This milestone comes under the leadership of CEO Isaac Wiener, and administrator Avi Terebelo overseeing the facility as it continues focusing on clinical standards, resident care and overall quality improvement.
Five Towns Premier, located at 1050 Central Avenue in Woodmere, is a 280-bed rehabilitation and skilled nursing center serving the Five Towns community. Its services include short-term rehabilitation, long-term skilled nursing and specialized care programs.
Facility leadership says the recently completed state survey was especially positive and reflects the center’s continued emphasis on regulatory compliance, resident care and operational performance.
Five Towns Premier is now looking ahead to how those results may affect its overall quality profile, with leadership expecting the center could move toward a 4- or 5-star rating. That rating has not yet been formally issued, so the potential upgrade remains an expectation rather than a confirmed designation.
Under Wiener’s leadership, the facility is continuing to build around a care model that emphasizes individualized treatment plans, interdisciplinary collaboration, family communication, dignity and cultural sensitivity.
The center also describes continuous quality improvement as a central part of its operations, with physicians, nurses, therapists, social workers and dietitians working together across resident care.
For nursing facilities, state survey performance is an important part of the regulatory and quality picture, but any change in a federal star rating ultimately depends on the methodology and data used by the Centers for Medicare & Medicaid Services.
